Form | Data Collection Setup Guide

Introduction: Feversocial's form feature comes in 3 types:

  1. Promo sign-up form: after creating a promo, build this in the [Sign-Up Form] section on the left side of that promo's editor. This form is automatically bound to the promo flow.

  2. Promo prize claim form: after creating a promo, build this in [Prize Editing] on the left side of that promo's editor. Once linked to a specific prize, this form opens up for winners of that prize to fill out.

  3. Standalone form: if you want to use the form system on its own, or simply want to add a form to a page, you can build a standalone form in the [Form] dashboard.

Step 1. Start building your form

Once you're in the form editor, we recommend renaming the form title first (click the small pencil icon in the top left) so it's easy to identify later.

  • Choose fields: the system provides preset fields (like name, phone number, email), so check the ones you need to collect.

  • Note: if you're collecting personal data, we recommend using a field from the encrypted field section — everything else can use a standard field.

Step 2. Configure field settings and data validation

  1. Edit the field: enter your survey question or a custom question title in each field you've added.

  2. Mark as required: turn on the required toggle if needed.

  3. Set duplicate checking: turn on duplicate checking if needed. This checks whether earlier respondents already submitted the exact same value. Limited to these fields: phone number & email.

  4. Choice-type fields — like text choice, image choice, dropdown, and two-tier dropdown — let you set a [Limit selection count] for each individual option.

  5. The two-tier dropdown supports bulk creation — please refer to the provided data template and prepare your upload file accordingly.

  6. The form's built-in validation automatically checks the format of certain fields.

    • Phone format: the phone number field automatically validates whether it's in a correct phone number format.

    • Email format: the email field automatically validates whether it contains an @ symbol and a domain name.

Step 3. Add a content block

You can add a content block anywhere on the form. Once added, it displays as a text block — use the text editor to add your content.

Step 4. Theme style

In the theme style section, you'll find fields for adjusting colors — as you make changes, you can see the form's colors update live.

Step 5. Form settings

In form settings, you'll find your form language setting and submission notifications.

  1. Form language: this setting changes the system's button text to your chosen language.

  2. Submission notification: set an email address where you'd like to receive notifications when the form is submitted.

Step 6. Hidden fields
